Our concierge model allows your doctors to spend more direct time with you and gives you a closer relationship with your physician and a better overall experience at an incredible value while still allowing you to use your insurance benefits.
Email contact directly with your physician and priority scheduling with your Connect & Thrive plan can give you the peace of mind that your doctor will be there to support you when you need it and not 1-2 months down the road.
At CIM, preventative and personalized medicine is always the norm and not an exception.
Concierge Plans
CONNECT & THRIVE (C&T) PLAN
This is our premium plan that allows CIM patients direct contact with their chosen provider through email, phone, and priority scheduling. While providing many discounts, this plan allows personalized care in an impersonalized medical system. While the insurance industry would never pay for this level of care, we hope to bring this service to you for a low monthly cost.
Also, a portion of your C&T plan is always donated to charity. This plan is the best for patients that utilize our non-billable services like 10-pass ozone, prolozone injection therapy, IV therapy, and/or do not have an insurance plan or have an out-of-network insurance plan.

BENEFITS FOR CONNECT & THRIVE PLAN
- Comprehensive 45-60 minute appointments with your personal doctor to address all of your concerns.
- Our Connect & Thrive patients have access to their providers 24/7 via email or phone at no charge.
- Priority booking times (8-9 AM, lunchtime, 4 PM to close), same or next day appointments if needed (excluding clinic closures).
- 12% Off all cash procedures & products including IV, Ozone, and injection therapies.
- 24% Off all-cash primary care service for those who do not have billable health care insurance.
- Yearly Vitamin D testing ($90 value)
- Free monthly B12 shots ($30/month value)
- Email access with a prompt response within 48 hours from your chosen doctor.
- Video and phone appointments while traveling.
- No charges for late cancellation or “no-show” appointments.
- Health Education, Articles, and Newsletters written by CIM doctors.
Concierge Plans
THRIVE (T) PLAN
Although we count this as our baseline plan, it is still far from the norm. The Thrive (T) plan allows us to continue to offer high-quality care, while it offsets and mitigates the rising cost of providing health care not paid for by insurance such as longer appointments with your doctor (30-45 minute office visits), lifestyle, and diet advice, insurance verification and authorization calls, email contact with your provider and other non-billable care.

BENEFITS FOR THRIVE PLAN
- Extended appointments to address all of your concerns (30-45 minutes appointments, instead of the traditional 10-minute visits).
- Access to your provider via email to address simple questions or clarifications.
- Access to your provider via telemedicine when traveling.
- Collaborative care with all CIM providers.
- Yearly Vitamin D testing ($90 value)
- Free support to help navigate your health concerns such as labs, prescription refill, and billing.

Concierge Plans
CONNECT (C) PLAN
This option is best suited for patients who choose not to participate in either Connect & Thrive or Thrive options. This option is best for patients who plan to come in no more than two to three times a year. Our Connect concierge patients still get the same benefit as our Thrive concierge patients, however, will pay a $34 per visit administrative fee at the time of service and only have access to Thrive concierge benefits for 2 weeks following your appointment. There is no couple or children package for this plan. This plan allows you to see our amazing doctors at CIM without the monthly payment plan.
- $34 time of service fee as well as any applicable insurance co-pays and deductibles.
While we recommend that all of our patients select one of the Concierge options, we understand that some patients would prefer to simply pay at the time of service for the high-quality care we provide at CIM. If this is your preferred option, you simply let the front desk know when you are scheduling or checking out.
If you change your mind, you can always set up one of the Concierge Wellness Plans whenever you wish.
